Industrial Radiography Permit

This permit is required for any industrial radiography operation executed within the University of Maryland campus. It must be completed by a competent industrial radiography supervisor (typically the service provider's Radiation Safety Officer) and posted at the site. If any of the conditions stipulated below cannot be satisfied, the work will not be permitted.

Conditions
  1. No work shall be conducted, or equipment brought to the work area, until the work area has been inspected and a permit has been issued.
  2. Operations shall be restricted to a designated controlled area
  3. The boundary containing the controlled area must be physically delineated in a clear and obvious way, for example cordoned off. Warning signs containing the standard Radiation trefoil must be prese
  4. The perimeter of the boundary must be under continuous surveillance during an exposure.
  5. At least two radiographers or one radiographer and one radiographer’s assistant must carry out the work.
  6. Only experienced, trained and qualified personnel shall be used.
  7. Each radiographer and radiographer’s assistant must be equipped with a survey meter and digital dosimeter.
  8. After each exposure, a survey must be done to determine that the sealed source has returned to its shielded position in the case of a gamma source or, for the case of an x-ray producing unit, that the beam is actually turned off.
  9. For the case of a gamma source unit, the entire circumference of the device must be surveyed as well as the entire length of the guide tube.
  10. Each time a gamma source unit is to be stored or left unattended, a survey must be performed to ensure that the source is in its shielded position. In addition, the source container must be locked. If an x-ray producing unit is to be left unattended, a survey must be done to ensure that the beam is off and the control panel must be set such that unauthorized use is prevented.
  11. The gamma source unit is to be outfitted with:
    • a warning label indicating that hazardous gamma rays are emitted when in operation
    • a warning label prohibiting unauthorized use
    • the radiation trefoil
    • a key switch to prevent unauthorized use
    • separate warning lights to indicate when the machine is energized and when gamma-rays are produced
    • a timer to control gamma-ray duration
  12. For the case of an x-ray producing unit, the tube assembly must remain stable during operation.
  13. The control panel of an x-ray unit is to be outfitted with:
    • a warning label indicating that hazardous gamma rays are emitted when in operation
    • a warning label prohibiting unauthorized use
    • the radiation trefoil
    • a key switch to prevent unauthorized use
    • separate warning lights to indicate when the machine is energized and when gamma-rays are produced
    • a timer to control gamma-ray duration

An emergency kit must be available, including, but not limited to, a high range survey instrument, a first aid kit, a portable communications radio, appropriate shielding, tongs at least 1.5 m long, hand tools, radiation warning labels and signs and plastic wrap to prevent contamination of instruments.
  18. No hot work is permitted near radioactive sources.
  19. In addition to the conditions specified for this permit, all state regulations regarding industrial radiology as specified in COMAR must be adhered to at all times.

Neither this permit nor any act or omission of the University of Maryland shall be construed to impair the obligation of any person under or in connection with any agreement with the University of Maryland or connection with the work done.

The contractor shall not do or permit to be done any act or thing which will invalidate or conflict with any insurance policy covering any area which the contractor enters upon, and the contractor shall promptly observe, comply with and execute the provisions of any and all present and future rules, regulations, requirements, orders, and directions of the MDE (COMAR Section E), or of any other board or organization exercising, or which may exercise, similar functions which may pertain or apply to the operations of the contractor hereunder.

The contractor understands that certain areas owned or leased to the University of Maryland are in turn leased or licensed to third parties. Without in any way limiting the conditions, provisions, and requirements in this permit, if the contractor requires entrance upon an area leased or under license to a third party, the contractor shall make independent arrangements for such entry with any such lessee or licensee.

The contractor shall observe and obey (and compel its officers, members, employees, agents, subcontractors, and other persons doing business with it to observe and obey) the present and future "Rules and Regulations" of the University of Maryland.

This permit is subject to cancellation by the University of Maryland immediately upon oral or written notice, in the opinion of the University of Maryland, such action is deemed justifiable or any reason whatsoever.
